Social Services Coordinator Job Description

Social services coordinators provide professional social work services and provide social work program support to the medical services provided by health departments. 

Typical Job Duties

• Interview patients in their home or in a health clinic in order to conduct a social assessment

• Determine the resource needs of the patient

• Determine psychological needs of the patient as well as any history of substance or physical abuse

• Confer with nurses and other support staff to prepare treatment plan for patient based on the social assessment

• Assist patient in acquiring social services within the community

• Visit home of patient as necessary to assist with social, emotional, financial and housing problems

• Follow up to monitor the provision of services and determine eligibility for selected services
